Paramedics said it was miraculous that the drivers involved escaped serious injuries in the Oxley Highway crash, near Somerton.
Six ambulances were deployed to the scene, near Appleby Lane, to treat multiple patients, just after 3.30pm.
Paramedics treated four patients at the scene including a 28-year-old man with head lacerations. He was trapped in the wreckage of the truck for a short time, but was rescued and released.
A 33-year-old male was also treated at the scene for minor head injuries; while a 37-year-old man suffered chest pain in the accident. A 50-year-old man suffered neck pain and was treated at the scene.
All four patients were taken to Tamworth hospital in a stable condition.
Ambulance NSW Duty Operation Manager, Inspector Paul McRae, said it was a confronting scene with five cars and a semi-trailer involved.
"Remarkably, all patients were transported to hospital in a stable condition," he said.
"Paramedics did a great job responding to the scene and treating all patients efficiently - considering the circumstances, this could have been much worse."
The highway has reopened, the RMS said.
One lane of the Oxley Highway was opened with traffic controls in place to get the traffic moving, while emergency services cleared the wrecks and debris on the road.
Both lanes are now open and there are no delays for motorists.
Investigations into the crash are continuing.

THE Oxley Highway is closed between Tamworth and Gunnedah after a serious accident involving multiple cars and a truck on Monday afternoon.
Tamworth police confirmed several police cars had been called to the bridge outside of Somerton, on the Oxley Highway, after reports of a crash about 3.45pm.
Ambulance NSW and police said crews had reported five cars and a semi-trailer had been involved in the accident, near Appleby Lane.
Six ambulances have been deployed to the crash scene with a truck driver, believed to be a man in his late 20s, still trapped in the wreckage.
Paramedics are also treating multiple other patients for minor injuries, the spokesperson said.
Police have blocked the Oxley Highway at Babbinboon Road, and also on the Tamworth side, near Appleby Lane.
There are lengthy delays with the road expected to remain closed for sometime.
Motorists travelling from Gunnedah will need to divert to Rushes Creek Road toward Manilla, police said.
Drivers are also being urged to use the Kamilaroi Highway, the RMS warned.
Investigations into the cause of the crash are continuing.
