The deadly fire broke out about 5.30am at a house on Bylong Road in Hillvue.
Police said a woman has suffered burns to her body and a child has suffered smoke inhalation, and both were taken to Tamworth hospital, but Ambulance NSW said four patients were assessed at the scene.
"Paramedics treated four patients including two children believed to be age five and seven, a woman in her 20s and an elderly woman," an Ambulance NSW spokesperson said.

"The adults and a five-year-old are suffering from extensive burns. The seven-year-old is being treated for minor burns and smoke inhalation."
Police have confirmed are man is assisting with their inquiries.
Dozens of police, detectives and fire crews are still combing the scene at the weatherboard home, after a crime scene was declared.
Bylong Road has been closed between Werris Creek Road and Warwick Road, and people are being urged to avoid the area.
